Every person receiving funds from Child Care Aware of Minnesota, through grants, scholarships, or payment for services, must complete a new W-9 each year.

• Download and complete a Federal W-9 form. The W-9 must be signed and dated or we cannot accept it.
• Fill out the electronic form below and upload your completed W-9 at the end. You will receive an automatic email reply once you have submitted the form successfully.
