T.E.A.C.H. MINNESOTA uses the information on the Pre-Authorization form to set up payment for your classes with the college(s) you are attending. The T.E.A.C.H. counselors will email you a copy of the payment letter once it is sent to your college(s). Complete and submit a new form for each term. Below, fill the course information for each class you would like T.E.A.C.H. scholarship to pay for this term. Be sure to include your student ID number for each college where you are taking classes. If your classes change, please complete a new Pre-Authorization form and submit it.
