This Book Reimbursement Forms needs to be completed and submitted each term in order to receive a reimbursement for your books. T.E.A.C.H. will not reimburse for books without a copy of a book receipt. Only required textbooks are covered. Complete this form by listing the full titles of the books you bought and the price for each (you may include shipping costs). Calculate the total price for books and list the amount. Upload copies of your book receipts before submitting.
